To find the best mattress for back pain, we focused on models with top feedback from users with recurring pain, consulted physicians and reviewed scientific literature.

Mattress shopping is tricky enough, but it's even more complicated if you're factoring in back pain. Here's what you should consider as you pick out your new bed, according to medical experts and Good Housekeeping Institute shopping pros:

  • Memory foam mattresses and hybrid mattresses are both great options, according to Jaspal R. Singh, M.D., co-director of the Center for Comprehensive Spine Care at Weill Cornell Medicine and New York-Presbyterian. There isn’t one definitive type of mattress that’s best for back pain, but patients who sleep on a mattress with some type of memory foam often report a more supportive and restful sleep, since memory foam offers both back support and pressure relief, especially around the hips and shoulders.
  • For those who don't like memory foam, don't worry. While it certainly offers benefits, memory foam isn't necessarily superior to other types of mattresses like innerspring or latex, which are known for their responsive support. "Patients should select mattresses that provide adequate support to their back. Memory foam vs. traditional is a matter of individual preference. Medically one is not superior to the other," says Amit Jain, M.D., M.B.A., chief of minimally-invasive spine surgery and associate professor of orthopedic surgery at Johns Hopkins University School of Medicine.
  • Medium firmness is effective in improving pain, according to Naimish Baxi, M.D., physiatrist at the Hospital for Special Surgery as well as a recent review of many medical studies. Plus, medium firmness offers an ideal balance of cushioning and support for side sleepers. A too-firm mattress doesn't let your shoulders or hips sink into the mattress, causing inadequate support of the neck and shoulders. On the other hand, if it’s too soft, there won’t be enough support, causing the hips and shoulders to sink in too much, resulting in improper spinal alignment.
  • A trial period is especially important for someone with back pain who struggles to find a comfortable bed. Many brands offer returns for at least 100 days so you can sleep on the mattress for a few months and get a refund if it's not a good fit. What is the best sleep position for back pain?

Side sleeping is often recommended if you have back pain, especially if you have pre-existing back problems. Dr. Singh even recommends side sleeping with a pillow between your knees, which will help take pressure off the lumbar spine.

That being said, the most important thing to do for back pain while sleeping is to keep your spine in neutral alignment. If you have lower back pain, sleeping on your back can “help alleviate pressure through the lower lumbar discs,” says Dr. Baxi. He also warns, “Sleeping on your stomach is generally not ideal due to the poor spine alignment when turning your head to one side.”

What is spinal alignment and why does it matter?

According to Dr. Jain, "Spinal alignment refers to the ability of the head to be spatially located on top of the pelvis to minimize the energy expenditure for the body." A neutral spinal alignment can be vital, especially for those with existing pain. If sleepers spend too much energy going against the natural curve of their body, that effort can result in additional back pain.

Similarly, it's crucial that your mattress and pillow accommodate the position of your spine, providing just enough comfort and support to maintain it.

Can a mattress actually get rid of back pain?

It depends. Chances are, your pain is caused by an underlying issue beyond your mattress, yet buying the right mattress can certainly be one factor in eliminating your back pain.

Dr. Baxi says, “Back pain can be complicated and has many other contributing issues. For someone with chronic lower back pain, mattress characteristics can contribute to reduced pain throughout the day." In fact, he said some studies have shown that the right mattress can improve pain, stiffness and sleep quality by up to 60%. For individuals with frequent neck and upper back pain, the height of your pillow may be another factor to consider.

That being said, it's a good idea to consider what else can be causing your pain. “It’s mostly related to what we do during the day that dictates the impact on our spine,” says Dr. Singh. He adds that one example is sitting at a desk all day, which puts pressure on the back and can cause lower back and neck pain.

If you find that you're experiencing recurring pain upon waking, it's best to consult with a physician to identify the nature of the pain, as a new mattress may not fully address any pre-existing conditions.

What to keep in mind when shopping for a mattress

Besides factoring in materials, sleep position and the trial period, it's important to consider other aspects unrelated to back pain. Here's what to keep in mind as you shop:

Lexie Sachs (she/her) is the executive director of strategy and operations at the Good Housekeeping Institute and a lead reviewer of products in the bedding, travel, lifestyle, home furnishings and apparel spaces. She has over 15 years of experience in the consumer products industry and a degree in fiber science from Cornell University. Lexie serves as an expert source both within Good Housekeeping and other media outlets, regularly appearing on national broadcast TV segments. Prior to joining GH in 2013, Lexie worked in merchandising and product development in the fashion and home industries.

Cynthia Stein, M.D. is a sports medicine physician at Boston Children's Hospital, where she sees patients of all ages. She is the director of the Medical Sports Medicine Fellowship and an assistant professor of orthopedic surgery at Harvard Medical School. She received her M.D. from UCLA and has a master’s degree in public health and another in medical education.  In addition to her clinical work, she is involved in teaching and research. She’s especially interested in concussion management, overuse injuries in performing arts athletes, and injury prevention.
